We conduct direct outreach and provide hygiene kits, essential supplies, and resource referrals to individuals experiencing homelessness.

Our case management services provide personalized support plans, goal setting, and ongoing guidance toward long-term stability and independence.

We connect individuals to essential health services including mental health referrals, substance use resources, primary care providers, and community-based wellness programs. We help remove barriers to care by providing guidance and support throughout the referral process.

We assist individuals in obtaining vital documents such as California state identification cards and birth certificates. We also help coordinate access to transportation resources, including bus passes, to support employment, housing appointments, court obligations, and essential services.
